How to deactivate my obsoleted mobile device for "Mobile Token" authentication?

Answer:
1) Login Two Factor Authentication Service Portal with your CNA account.
2) Input one-time password by SMS or Mobile Token.
3) Click "Activate / Deactivate Mobile App"
4) Find out the Device and Click "Deactivate"
5) Deactivate completed.
